Request Invoice Copy
To request a copy of your invoice, contact the USAC Customer Operations team via email with the following information included in the body of the email:
- Requestor’s name
- Filer ID
- Date of statement
If your name is not listed on the current FCC Form 499-A, FCC Form 499-Q, or current invoice, USAC cannot provide you with a copy of an invoice.
If you are listed on one of these forms, and you include the above information, you should receive a copy of your invoice via email within 24-48 hours of the time the request was received by USAC.
