Submit Annual Report
All HCPs participating in HCF must submit an annual report to USAC that includes information allowing the Federal Communication Commission (FCC) to assess progress towards the performance goals and measures of the Healthcare Connect Fund (HCF) program. The health care provider (HCP) must file the annual report on or before September 30 for the preceding funding year.
FY2022 Annual Reporting Requirement
For Funding Year (FY) 2022, to supplement the information collected from forms filed by the HCP, the FCC requires HCF participants who received funding in FY2022 (July 1, 2022 – June 30, 2023) to submit information to USAC about the telehealth applications used during the funding year.
How to Submit the FY2022 Annual Report Questionnaire:
- Open the FY2022 Annual Report Template.
- In the “Telehealth Services” tab, review the telehealth applications in column A and use the dropdown menu in column B to select “Yes” for each telehealth application for which the HCP or consortium received funding in FY2022.
- Save the template with a file name that includes the HCP or consortium name and HCP number.
- Email the completed template to RHC-Outreach@usac.org by September 30, 2023. Please include the HCP name and number in the subject line of the email and in the file name of the template.
Failure to submit the required annual report(s) by the deadline may result in the denial of program funding.
