November 1, 2021

The November FCC Form 499-Q is due on Monday, November 1, 2021. Service providers are required to project their 1st Quarter 2022 telecommunications revenue on this form.

While this form is not a required filing for de minimis service providers, USAC highly encourages them to file the form anyway.

November FCC Form 499-Q Revision Deadline

Revisions to the November 2021 FCC Form 499-Q must be submitted to USAC by December 16, 2021.

Any revisions submitted after the deadline will not be used for purposes of determining federal USF contribution levels.
