Eligible telecommunications carriers (ETCs) participating in the High Cost program must file FCC Form 481 report with USAC on an annual basis. This form collects financial and operations information used to validate carrier support and fulfills the annual 54.313 certification requirement.

Carriers must submit their second quarter 2026 performance measures testing results.

Annual 54.314 Certification Due
Eligible telecommunications carriers (ETCs) are required to be certified to receive High Cost program support, pursuant to Section 54.314, including certification of Mobility Fund Phase I and Tribal study area codes.  Most carriers are certified by their states, but carriers not subject to state jurisdiction must self-certify. CAF BLS Form 509 Actual Data Due
Recipients of CAF BLS must submit their actual cost and revenue data, including actual voice and broadband-only loops on Form 509 for Calendar Year 2025.

Mixed Merger Certification
Carriers that have been part of a “mixed merger” must submit the latest audited financial statements, including all notes and consolidating statements, and certify that it has complied with the “Hargray condition”. Learn more
