Application Status Report

New Application Status Report Tool

Use the Application Status Report tool to learn if USAC has received your FCC Form 466 and collected the information it needs to make a funding commitment decision. If we are missing information, you will receive an Information Request once your form has been assigned to a reviewer.

The tool’s dataset refreshes daily. If your form status does not appear in the report more than 5 days after submitting your FCC Form 466 in My Portal, please contact the RHC Customer Service Center via email or at (800) 453-1546.

Note: Beginning with FY2012, “packets” will be referred to as “forms” and “Packet Status Reports” will be referred to as “Status Reports.”

Status Codes – FY2012 and After

Approved:
All forms in this group have been issued a funding commitment letter (FCL) or will be issued one shortly.

Denied:
All forms in this group have been denied.

Duplicate:
All forms in this group are duplicate submissions and will not be processed.

In review:
All forms in this group are currently being processed.

Pending – waiting for applicant info:
All forms in this group are awaiting the applicant’s response.

Received:
All forms in this group have had an initial cursory review for completeness and are in the queue to be processed.

Withdrawn:
All forms in this group have been withdrawn and will not be processed.

Status Codes – FY2011 and Before

In process:
All packets in this group have had an initial cursory review for completeness (which includes forms, signatures, etc.) and are in the queue to be processed. Applicants are notified if their packets are incomplete.

Requires forms or clarifications:
All packets in this group are either missing entire forms, required information on a form, or the information provided is unclear. For each of these packets, either the Health Care Provider (HCP) or the service provider is contacted by email, phone, fax, or letter describing the information needed to complete the processing of the packet.

Processing Complete:
All packets in this group have been issued a Funding Commitment Letter (FCL) or will be issued one shortly. Withdrawn packets are also counted in this group.
