My Portal
All forms for the Rural Health Care program are filed through the online application management system My Portal. Access My Portal via the E-File system by clicking the “My Portal” button below.
Accessing My Portal
A My Portal account is created by submitting an FCC Form 460 (HCF program) or Form 465 (Telecom program). Upon approval of the form, you will receive an automatic email with instructions for creating your My Portal password.
Multi-Factor Authentication
In December 2019, USAC implemented multi-factor authentication to increase security of most IT applications. MFA helps safeguard access to data and applications, and provides additional security by requiring a second form of user authentication. Learn more.
Forgot Your Password?
Reset your password by clicking “Forgot Password?” on the E-File login page. Your username is your email address.
Account Holder Types
Primary Account Holder: Each HCP has one primary account holder. The primary account holder has the ability to sign, certify, and submit forms for an HCP. The primary account holder must be a director, officer, or authorized employee of the HCP.
Secondary Account Holder: A secondary account holder also has the ability to sign, certify, and submit forms for an HCP. You can designate multiple secondary holders on your FCC Form 460 and/or 465. For the HCF program, the secondary account holder must be a director, officer, or authorized employee of the HCP. USAC recommends assigning at least one secondary account holder so that the HCP is still able to submit forms and meet deadlines should the primary account holder be unavailable.
Tertiary Account Holder (HCF Program Only): Tertiary account holders are third parties/consultants authorized to submit forms on behalf of an HCP. The third party must submit a Third Party Authorization before they can access My Portal.
My Portal for Service Providers
Service providers have the ability to submit and manage all invoices using the service provider section in My Portal. For assistance getting access, contact USAC Customer Operations at (888) 641-8722 with your 498 ID (formerly known as SPIN).
