Send Mail to USAC
The easiest way to contact USAC is via email, but we understand sometimes you need to send us a letter or package. All FCC forms should be mailed to the address listed on the form. Lifeline Program applications should be sent to the telecommunications company directly.
If you participate in the Lifeline Program through your telecommunications company and have questions or problems with your service, want to report Lifeline Program fraud, or have a complaint about your telecommunications company, please visit the Help with Lifeline page.
