FCC Form 473 Filing

Service providers must file the FCC Form 473, Service Provider Annual Certification (SPAC) Form each year that they provide services under the program. Service providers certify each year on the FCC Form 473 that they will comply with program rules.

Purpose of Form

Service providers file an FCC Form 473 (SPAC Form) to certify that they will comply with program rules. USAC must have a SPAC Form on file for each Service Provider Identification Number (SPIN) - if a service provider has more than one - and for each funding year.

When to File

Service providers can file SPAC Forms on or shortly after the application filing window opens for the upcoming funding year.

Deadline: A SPAC Form must be on file with USAC before USAC will pay invoices - FCC Forms 472 and FCC Forms 474 - for a funding year. Refer to invoice deadlines and extension requests for more information on invoice deadlines.

After Filing

The search for SPIN information tool will show all funding years for which USAC has successfully processed an FRN in the far-right column of the search results.
