Customer Service Center Documentation
This page lists the documentation and information that E-Rate applicants should gather when calling the E-Rate Customer Service Center for certain situations. Please contact us if you are uncertain about any of the information below. Please click the + (plus) button to expand the selection.
- Entity name
- Physical address, including street, city, county, state, and zip code (4-digit zip code extension is optional)
- Mailing address or PO Box (if different than physical address)
- Telephone number
- Sub-type: public or private
- FCC Registration Number (for all independent entities that are not part of a consortium)
- Optional: email address and website
- If you are an employee of the organization making the request for a new entity, provide the written request on entity letterhead, or send an email through the applicant entity’s domain.
- If you are a consultant making the request for a new entity, please provide the following:
- Written request on applicant letterhead
- Letter of Agency (LOA) to demonstrate that they are authorized to act as a consultant on the entity’s behalf. If the consultant user has an account under the parent entity or consortium, then an LOA is not required.
- For all new parent entities, an Account Administrator would also need to be establish at the time of entity creation.
- To establish an Account Administrator, provide a signed written request from the new administrator containing the Billed Entity Number (BEN), the administrator’s name, job title, address, phone number, and email address.
- If the new school is a part of a district, please provide the district entity number
- If it is an independent school, provide the name and entity
- All additional school sub-types that apply:
- Pre-K, Head Start, adult education, juvenile justice, dormitory, charter school, Tribal school, new construction school, Educational Service Agency (ESA) schools, Bureau of Indian Education, ESA school district with no schools
- If Tribal sub-type is selected, provide the Tribal affiliation
- For Districts: Charter school district or ESA school district
- Information on student populations for calculating discounts and C2 Budgets
- Total number of full-time students
- For Districts: Provide either a number for each school in the district or one number for the whole district
- Total students eligible for the National School Lunch Program (NSLP)
- Alternative Discount Method: none, survey, sibling match, combination (not a Community Eligibility Program (CEP))
- CEP percentage and base year (if applicable)
- Total number of full-time students
- Current value of endowment (if applicable)
- Optional Public or Private School Codes:
- State School, State LEA, NCES code (public state, public district, public building, or private school ID)
- If the library is part of a library system, provide the library system’s entity number
- If it is an independent library, provide the name and entity number (if known) of the school district where the library is located
- Provide additional library sub-types that may apply:
- Academic, research, Tribal library, bookmobile, kiosk, new construction library, main branch, State Library Agency’s main branch
- If Tribal library sub-type is selected, provide Tribal affiliation
- Library’s square footage
- Optional: Locale Code, FSCS codes
Creating a New Entity – Additional Information to Gather (if Applicable) for Consortia
- Consortium system sub-types that may apply:
- State-wide and ESA consortium
- If you are an employee of the organization making the request for a new consortium entity, provide the written request on letterhead, or send an email through the entity’s domain
- If you are a consultant making the request for a new entity, please provide the following:
- Written request on applicant letterhead
- Letter of Agency (LOA) to demonstrate that they are authorized to act as a consultant on the entity’s behalf. If the consultant user has an account under the parent entity or consortium, then an LOA is not required.
Adding or Updating Applicant Account Administrators
- The Account Administrator must be an employee of the entity and cannot be a representative from a consulting firm.
- Employees of the organization may add or update their account administrator by providing a signed written request from the new administrator containing the Billed Entity Number (BEN), the administrator’s name, job title, address, phone number, and email address.
- The new administrator can email from the entity’s domain, or provide a written request on the entity’s letterhead.
- Consultants may add or update the applicant account administrator by providing a signed written request from the new administrator containing the Billed Entity Number (BEN), the administrator’s name, job title, address, phone number, and email address on the applicant entity’s letterhead.
- In addition to the written request, consultants will need to provide a Letter of Agency (LOA) to demonstrate that they are authorized to act as a consultant on the entity’s behalf.
- Because the account administrator cannot be a consultant, the email address provided in the written request cannot contain a consultant’s name or consultant’s email domain.
- If there is already an Account Administrator established for this entity, please include a brief statement in the request that the current Account Administrator is absent or no longer with the entity.
- A service provider Account Administrator change requires an FCC Form 498 update.
Please wait until all invoicing disbursements have been received to submit an entity closure request.
To close an entity, please have:
- Name of the entity
- Billed Entity Number (BEN)
- Date the entity closed
- Reason the entity closed
- Documentation that indicates the entity is closed, for example public records from reputable sources such as Board of Director minutes or newspaper articles
- Name, title, and contact information of the person making the request
Merging entities is when two or more entities consolidate to form a single entity. For example, in the case of schools, one school closes and another school absorbs the student population.
- Merger requests can be submitted at the same time as your entity closure request.
- Please wait until all invoicing disbursements have been received for each entity before submitting a merger request.
If the entity has closed and invoicing is complete, please provide the following information regarding the merger:
- Entity name(s) and BEN(s) of the entity(s), pre-consolidation
- Name(s) and address(es) of the new consolidated entity(s)
- Effective date of merger
- Concise, clear description of the merger/consolidation
- Documentation of the consolidation from the state that shows the former entities, the new entity(s), and the date of the consolidation
If feasible, please provide the following documentation for reporting an EPC system issue.
- Text, screenshot, and/or printout of the error message
- Operating system and version (e.g. Windows 10, Mac OS X)
- Web browser and version (e.g. Internet Explorer 11)
- Notate the following:
- Which EPC account were you using when the issue occurred?
- Where within EPC did the error or issue occur? For instance, which tool, screen, field in a form, etc.?
- What happened just before the error occurred? For instance, did you click a button, hit tab, enter data into a field, etc.?
- If the error occurred within a form, please also provide the following:
- FCC Form Number/Type
- Specific Application Number, PC Request Number, or Invoice Line Number
