Form 481 Due
Eligible telecommunications carriers (ETCs) participating in the High Cost program must file FCC Form 481 report with USAC on an annual basis. This form collects financial and operations information used to validate carrier support and fulfills the annual 54.313 certification requirement.

Eligible telecommunications carriers (ETCs) are required to be certified to receive High Cost program support, pursuant to Section 54.314, including certification of Mobility Fund Phase I and Tribal study area codes.  Most carriers are certified by their states, but carriers not subject to state jurisdiction must self-certify.
