Submit Annual Report

All HCPs participating in HCF must submit an annual report to USAC that includes information allowing the FCC to assess progress towards the performance goals and measures of the Healthcare Connect Fund (HCF) program. The HCP must file the annual report on or before September 30 for the preceding funding year.

FY2021 Annual Reporting Requirement

For Funding Year (FY) 2021, to supplement the information collected from forms filed by the HCP, the FCC requires HCF participants who received funding in FY2021 (July 1, 2021 – June 30, 2022) to submit information to USAC about the telehealth applications used during the funding year.

How to Submit the FY2021 Annual Report Questionnaire:

  1. Open the FY2021 Annual Report Template.
  2. In the “Telehealth Services” tab, review the telehealth applications in column A and use the dropdown menu in column B to select “Yes” for each telehealth application for which the consortium received funding in FY2020.
  3. Save the template with a file name that includes the HCP or consortium name and HCP number.
  4. Email the completed template to with “Annual Report” in the subject line, by September 30, 2022.

Failure to submit the required annual report(s) by the deadline may result in the denial of program funding.
