Healthcare Connect Fund (HCF) Invoicing Deadline for FY2015

You must submit all your FCC Forms 463 to USAC within 6 months of the end date of your funding commitment. So for FY2015 funding commitment that ended June 30, 2016, a corresponding FCC Form 463 must be submitted to USAC by December 31, 2016.


You can find your invoicing deadline on your funding commitment letter (FCL), which USAC emailed you after approving your FCC Form 462. Alternatively, you can look up an invoicing deadline by visiting the Initiate Invoicing page and downloading the Invoicing Deadlines Excel spreadsheet.


After you complete your FCC Form 463, you must then submit it to your service provider for review. When the service provider approves the form, it is automatically submitted to USAC. Start this process early to allow time for the approval process with your service provider and for revisions.


Note: This deadline is for FY2015 for the HCF Program only and does not apply to the Telecom Program.

FY2016 Funding Requests – Update?

The RHC team continues to review FCC Forms 462 and 466 that were submitted during the second filing window period (September 1 – November 30) and have begun issuing funding decisions.

Skilled Nursing Facilities Eligible for Funding – Apply Starting January 1?

New legislation adds skilled nursing facilities (SNFs) as an eligible entity for RHC Program funding. SNFs can apply for funding starting January 1, 2017.

Website Refresh?

We gave a fresh, new look. This was a small first step in the process to offer clearer, more streamlined and intuitive experiences for you, making universal service programs simpler to access and in turn maximizing the impact of the funds.


Our goal is to bring clarity to what can be a complex process, but we know this will not happen overnight. Here is a glimpse of what to expect:


What is different?

For now, we simply changed the visual look and the organization of the home page. The navigation on the subpages is now simpler, allowing you to switch easily between programs.


Has the content changed?

No, the content of the website has not changed. You will still find all the tools, resources, and information that you need in the same place. In the coming months, you will begin to see some adjustments to the content, including getting rid of information that is out of date.

Forms Enhancements

Starting January 1, 2017, the HCF and Telecom Programs application forms will feature a number of enhancements to further align the two programs. You will notice minor changes. These new fields promote alignment between the programs, ensuring that the same data is collected, such as a new entry field for your organization's website and document uploading capabilities.


These changes will apply to both FY2016 and FY2017 forms. If you started a form in calendar year 2016, the system will automatically pre-populate the new forms with your saved information on January 1, 2017, but you may have to enter additional information for new mandatory fields before submitting.


Best Practices & Tips

LOA/TPA Reminder 

Third parties and consortium leaders are required to have current authorization documents on file to continue filing forms on behalf of health care providers. Email updated Third Party Authorizations (TPAs) and Letters of Agency (LOAs) to Include the health care provider's number(s) associated with each TPA or LOA in the body of the email. The new LOA/TPA should be re-executed and updated with a new authorization date.

Reminder to Respond to Information Requests Within 14 Days?

Failure to provide all required information when submitting a form will increase processing time and result in an information request. Information requests are emailed directly to the applicant. Applicants have 14 days to respond to an information request. If the information request is not responded to completely or in a timely manner, USAC will deny the funding request. It is very important to fill out your forms carefully to avoid potential errors that might result in an information request or denial of funding.
