2018 USAC-Elected Recertification 

USAC elections for 2018 recertifications open May 15! The process will begin September 2017 for January 2018 anniversary dates.

To elect USAC for the first time, complete the recertification election spreadsheet and submit it to If you elected USAC for 2017, re-election is automatic; resubmit your election spreadsheet only if your contact information has changed. 

Please note, service providers with multiple Study Area Codes (SACs) may choose to have USAC conduct recertifications for all or some of their SACs. Visit our recertification page for more information.

FCC Form 481 Due Date

This year, the FCC Form 481 is due July 3, 2017. In an effort to improve our processes, service providers no longer need to complete lines 500 and 510; however, they must complete line 515 (resource video) to certify their compliance with the minimum service standards. Service providers can continue to access the FCC Form 481 through E-File

Enhancements to NLAD Reports

Service providers can now select multiple Study Area Codes (SACs) when viewing their reports in NLAD. This functionality has been added to the Detail Active Subscriber Report and the Detail Subscriber Snapshot Report, which can be accessed under "Reports" on NLAD's left-hand navigation pane.


Security Upgrade

In the NLAD v3.4.5 release, we added the Completely Automated Public Turing test to tell Computers and Humans Apart (CAPTCHA) feature to the NLAD login page. This enhancement will improve security by requiring users to enter their NLAD username and password, and then confirm that they are human on the NLAD login page. This is an added security feature to help protect information.

We simplified our consumer website,, to provide better customer service to new and existing consumers. The new streamlined design is more intuitive and makes finding information easier for all stakeholders. The changes include: 

We've also included a Help page that explains common issues that customers may encounter and how to navigate toward a resolution. The When USAC Can Help section helps customers identify when to call their service provider. We revamped these pages based on common questions to reduce customer frustration by directing them to the appropriate resource.


Another addition to the website is our community focused section. The Community Outreach section features Spanish language brochures and postcards which are available for download and can be distributed to your consumers. We also encourage consumer advocacy groups to share these materials with their Spanish-speaking communities.

Some service providers' Lifeline Program subscriber certification and/or recertification documentation do not contain the required disclosures resulting in the audit finding "Improper Certification and/or Recertification Documentation Disclosures "


To prevent this, please include all certifications and disclosures on advertising materials, certification and recertification documentation. 


All prospective and existing subscribers must acknowledge each of the certifications on these types of documentation by individually initialing in each certification box. Please review this sample recertification form for the list of required certifications.
