File an FCC Form 460
Health care providers (HCPs) seeking to participate in the program must obtain an eligibility determination from USAC by filing an FCC Form 460. In a change from the first round of the Program, only the lead HCP listed on an application needs to complete the FCC Form 460 – the applicant will then certify that the other sites that could receive funding requested on the application are eligible for Round 2 of the COVID-19 Telehealth Program.
HCPs who have an active FCC Form 460 eligibility determination – from the first round of the COVID-19 Telehealth Program or from participation in the Universal Service Fund’s Rural Health Care program – can rely on that eligibility determination and do not need to file another FCC Form 460.
If you do not already have an active eligibility determination from USAC, you can obtain one by filing an FCC Form 460 through My Portal on USAC’s webpage. You can download and view a blank FCC Form 460 for informational purposes, but filing must be done through My Portal.
When an FCC Form 460 is filed, USAC will assign the HCP an HCP number. That HCP number must be included on the Round 2 COVID-19 Telehealth Program application. The HCP number is a unique identifier that USAC uses to maintain registration of eligible HCP sites and ineligible sites.

- If I apply as a health care system or include multiple HCP sites on my application, can I only file one FCC Form 460 for the lead HCP?
Yes. In the second round of COVID-19 Telehealth Program, only the lead HCP listed on an application needs to file an FCC Form 460 and obtain an approved eligibility determination. - Do I need to have a decision on my eligibility from USAC before I submit an application?
No, you do not. HCPs may submit their applications while their eligibility determinations from the FCC Form 460 are pending with USAC. However, no funding will be awarded to an applicant until the lead HCP site has been determined to be an eligible HCP by USAC. - What if I submitted an FCC Form 460 for Round 1 of the COVID-19 Telehealth Program, and I do not know if my HCP received an approved eligibility determination?
