Funding Request

1. Navigate to the Funding Request Tab.
2. The Total Amount of Funding Requested will be filled in for you – it automatically calculates all the funding request details entered into the application.
3. Indicate whether you are requesting funding for devices. If so, you will be prompted to indicate if these devices are for patient or provider use.
