Applying for the Affordable Connectivity Program

Consumers can submit a program application online at AffordableConnectivity.gov, with a service provider, or by completing the ACP paper application. The application is also available in Spanish.

USAC recommends consumers include completed copies of the ACP Household Worksheet (Spanish Version) along with copies of their identity and eligibility documents to speed up application processing.

Sending Proof of Eligibility, Identity, or Address

If USAC cannot find a consumer’s ACP eligibility, identity, or address in a database, they will need to show proof.

USAC will contact consumers to let them know what type of additional information they will need to submit. The type of information requested will relate to the consumer’s eligibility, identity, or address. Consumers who are attempting to resolve an error should visit our ACP consumer website.

Consumers can make copies or take pictures of the documents using a scanner, copy machine, camera, or smartphone. Copies or pictures should be clear and easy to read, and they should not be dark or blurry. Consumers should never submit original copies of their documents.

To submit copies, consumers can:

  • Log in to the National Verifier online portal and upload their copies
  • Visit their participating broadband provider’s store location and provide them with copies
  • Mail their copies to this address:

If consumers are mailing in documents, they should always include a cover letter with their application ID and name so that USAC can match their documents to their application.

Duplicate Address

To resolve a duplicate address error, consumers must prove they are an independent economic household by submitting a Household Worksheet (Spanish Version).

A household is a group of people who live together and share income and expenses (even if they are not related to each other). Consumers are only allowed one ACP benefit per household, not per person.
