To obtain a Service Provider Identification Number (SPIN), a service provider must complete the FCC Form 498 (Service Provider Identification Number and General Contact Information Form) and submit it to USAC for review and approval.
First-time service providers can file the FCC Form 498 using USAC's E-File system, but you must send an original, officer signed copy to USAC the first time. After the first filing, the Company Officer will be able to certify revisions online via the E-File system.

If you submitted your FCC Form 498 using E-File, you will receive a confirmation email notifying you that USAC received your submission. If you do not receive this email, check your spam and/or log back into E-File to verify that your form was submitted.
If you mailed in your paper FCC Form 498 to USAC and did not use E-File, you will not receive a confirmation of receipt. If USAC is unable to process your request due to missing documentation or information, USAC will email the General Contact listed on the form, explain the issue, and request resubmission.
For both paper forms and forms filed electronically, if all appropriate documentation and information is received by USAC, it takes approximately two weeks to review, process, and send out your new SPIN. After one week, you may contact USAC to find out the status of your SPIN application (FCC Form 498) by calling (888) 641-8722.
