The FCC Form 498 collects contact and remittance information for service providers that receive support from the federal universal service programs. Service providers should file FCC Form 498 when they are updating or changing any banking or contact information related to their service provider information on file.

FCC Form 498 Process (Paper Submission)

1. Service provider submits form to USAC
2. Form reviewed by USAC
3. Final approval of FCC Form 498 data and customer notification

FCC Form 498 Process (E-File Submission)

1. Service provider submits and certifies E-File data via E-File system
2. E-File validates and confirms the data
3. Final approval of FCC Form 498 data and customer notification (automated through E-File)
