Schools and Libraries Tools:
Billed Entity Applicant Reimbursement Form (Form 472)
All schools and libraries and consortia of those entities that have received a Funding Commitment Decisions Letter from USAC and that have already paid for the eligible services which are approved for discounts seek reimbursement of the discounts by filing the Form 472. Form 472 informs USAC of the amount of the discounts for which the applicant has already paid and for which the applicant seeks reimbursement from its service provider.
