The data collected on Form 471 is used to ensure that schools and libraries are receiving the appropriate discounts, complying with the eligibility requirements, and taking the required steps that are necessary to use the discounted services effectively. All schools and libraries ordering services eligible for universal service discounts must file this form, individually or as part of a consortium.
