Step 6: Electronic Notifications
Service providers can receive electronic notifications of certain data that are usually supplied in letter format.
Electronic Notification Procedures
USAC strongly encourages service providers to sign up to receive electronic notifications. Electronic notifications consist of electronic files of funding commitments, receipt of the Description of Services Ordered and Certification Form (Form 471) filed, electronic versions of the Receipt of Service Confirmation Form (Form 486) Notification Letter, the Adjustment to Funding Commitment and Modification to Receipt of Service Confirmation Form (Form 500) Notification Letter, and Invoice Status Reports.
Service providers interested in electronic notifications should send a message to SLDFORM@sl.universalservice.org indicating their SPIN and e-mail address where they wish to receive electronic notifications. The e-mail address should be in a generic form, such as: Notification@ABC.com, so that even if your company experiences personnel changes or staff is away on vacation, electronic notifications can continue to be processed in a timely fashion.
