Applicants should ensure that they indicate the proper category of service and service type for products and services specified in each funding request of the FCC Form 471 (Description of Services Ordered and Certification Form).
The Eligible Services List (ESL) provides information about the proper categories of service and service types. USAC carefully reviews the services for which funding is requested to ensure they are requested in the appropriate category of service or service type.
For instance, USAC reviews Category One services to ensure that voice services, which are subject to a phase down, are requested under the service type Voice Services so that the correct discount is applied. If applicants make errors in specifying the proper category of service or service type, USAC may adjust the category of service or service type subject to the rules of fixing Ministerial and Clerical Errors.
If an FRN request for Data Transmission Services and Internet Access is found to also contain charges for voice service, those charges for voice service and any circuit capacity dedicated to voice would be split and moved to a separate funding request.
If an FRN is split during PIA review, both the original and the new FRN will appear in the Funding Commitment Report of the Funding Commitment Decision Letter (FCDL) for the FCC Form 471 that contains the original FRN.
