FY2005 Forms 465, 466, 466-A Ready by July 8, 2005
RHCD plans to have the new FY2005 Forms 465, 466, and 466-A ready for use by Friday July 8, 2005. Please check the RHC web site for more information regarding the forms availability over the next few days. RHCD also expects to send out reminder emails to all HCP’s who have already submitted a FY2005 Form 465 to inform them that the NEW FY 2005 forms are available.
In addition, all applicants are reminded that they must provide FCC Registration number(s) in order for RHCD to process their application. For more information on the FCC Registration number requirement for FY 2005 please click here.

Applicants have set an all time record for the number of packets submitted for a single funding year. To date we have received over 4,500 applications for telephone and internet discounts. Of interest is the fact that RHCD received more packets in June 2005 than we did in the entire first year of the program, In addition, in the month of June, RHCD received nearly 25% of the total number packets submitted for FY 2004. RHCD would like to thank all HCPs for responding to the outreach postcards and submitting their Form 466 and 466As before the June 30th due date.
But, we need your help to finish the process..
Nearly half of all the packets received this month were missing contracts, bills, urban rates, SPINS, or other documentation that RHCD needs to complete its processing. These items are necessary in order to send out applicant Funding Commitment Letters. Please help us help you, by sending in the missing information as soon as possible. Also, please indicate on the top of this supporting documentation that this information is for Funding Year 2004 and write the Packet ID number and HCP number at the top of the document. This extra step will help our staff process your packet more quickly.
Over the next few weeks, RHCD will be contacting those HCPs whose packets
are incomplete, but don’t wait for us to call or write. Please send in
your contract, bill or other items now. Once again, thank you to everyone for
making this the most successful year of the program to date.

The Application Window for Funding Year 2005 (7/1/2005 – 6/30/2006) Is Now Open!
Over the next few weeks RHCD will be posting information on its website as well as mailing and emailing applicants and service providers about changes for FY2005. These changes will include new application forms, which are currently under review by OMB. However, to help ensure that applicants have plenty of time to post Form 465 for before the Funding Year starts July 1, 2005, RHCD has authorized the using the old Form 465 for Funding Year 2005.
How do I use the 2004 Form 465 for Funding year 2005? For returning applicants simply go to the Applicant Login Page, and select Funding Year 2005. A Form 465 with last year’s information will be pre-filled with your information.
For new applicants, please download the PDF or Excel spreadsheet on the Forms Page, sign and mail this to the Rural Health Care Division, 80 S. Jefferson Road, Whippany, NJ 07981.
Because RHCD continues to accept Form 465 postings for Funding Year 2004 as well, it is essential to select the correct Funding Year for which you are applying, in this case, FY 2005 (7/1/2005 – 6/30/2006). If you have questions please call us at

Second Report and Order, Order on Reconsideration, and Further Notice of Proposed Rulemaking Published in the Federal Register
The Commission has published a notice in the Federal Register modifying its rules to improve the effectiveness of the rural healthcare universal service support mechanism and seeking comment on whether it should increase the percentage discount that rural health care providers receive for Internet access. The Commission is also seeking comment on whether infrastructure development should be funded and whether its rules should be modified to allow mobile rural healthcare providers to use services other than satellite. Comments are due on or before April 8, 2005. Reply comments are due on or before May 9, 2005. For a copy of the entire listing in the register please click here.
