Step 3: Provide Bids for Requested Services

After USAC posts a health care provider's Description of Services Requested & Certification Form (Form 465), both telecommunications and Internet service providers may search for requests for services in specific counties or zip codes and provide bids.

The open bidding process is 28 days from the date USAC posts a Form 465 on USAC's website. During this 28-day window, service providers may contact the HCP to submit a formal bid for their service needs. Health care providers must evaluate all bids and select the most cost-effective service.

Service providers may use the Search Service Requests tool to search for requests for services in specific counties or zip codes.
