Step 2: Search Service Requests - Instructions
Below are instructions describing how to use the Search Service Requests Tool for the Rural Health Care Program.
The Search Service Requests tool allows you to search health care provider (HCP) requests for services from a posted Description of Services Requested & Certification Form (Form 465). Posted information includes:
- Services requested
- HCP's name, location, and point of contact
- Maximum allowable distance (MAD)
- largest city in the HCP's state.
Searches are performed by specifying any of the following information:
- HCP Number
- HCP Name
- City
- County
- State
- Request for New Services
- Date Posted: enter a range ("From" and "To") for the date posted. If you only enter the "From" date, all applications posted after that date are retrieved. If you only enter the "To" date, applications posted before that date are retrieved.
To locate posted information:
- Select a funding year from the drop down list on the Search Service Requests page.
- Enter information into any of the search fields.
- Click the Search button to start the search.
- Select a Form 465 from the list to view the complete form.
You may enter partial search information. For example, if you know that a name contains the word "Rural," enter "Rural" and all forms that contain the word "Rural" are retrieved.
