Step 7: Submit Confirmation of Service
The health care provider completes and submits the Connection Confirmation Form (Form 467) to USAC.
After the health care provider begins to receive service from a service provider, it submits the Connection Confirmation Form (Form 467) to USAC. The HCP must submit Form 467 in order to receive discounted services. USAC cannot process Form 467 unless a Funding Commitment Letter was issued. Form 467 is available for E-certification.
This form must also be used to notify USAC when the health care provider has discontinued the service (or the service was turned off), or that service was not (or will not be) turned on during the funding year. An applicant must submit one Form 467 for each Funding Request and Certification Form (Form 466) and/or Internet Service Funding Request and Certification Form (Form 466-A).
