How to apply

Applicants should file FCC Form 465 (Description of Services Requested and Certification) as soon as possible, but no later than December 13, 2005.  Please complete the form online or download at: http://rhc.universalservice.org/forms/default.asp#2005.  Whether the form is completed online or downloaded, a signed paper copy MUST be mailed to the address on the form. 

Note that to complete Form 465, applicants must have or register for an FCC Registration Number, which is a 10-digit number assigned to an entity or individual to uniquely identify the registrant in all transactions with the FCC.  For more information or to apply for and receive a number immediately online, please visit www.fcc.gov and click the link for FCC Registration Number (FRN) Commission Registration System (CORES).  Forms may also be downloaded to apply for a number by mail. 

Rural applicants that would be eligible without regard to these special provisions (see   http://rhc.universalservice.org/eligibility/applicants.asp) should complete Form 465 according to the instructions, but in addition to their service request on Line 29, add, “This is also a request for advanced telecom or information services for telemedicine or the provision of health care for victims of Hurricane Katrina”.  (Applicants already posted that wish to modify their posting should contact USAC to add this request.)  

Urban or other applicants eligible only under these special provisions may ignore the word “rural” in selecting health care provider type on Line 27, but on Line 35, should strike “rural” and write “Hurricane Katrina disaster”.   Red Cross or temporary shelters should provide a brief description of their set up on Line 28. 

On Line 29, all applicants should write, “This is a request for advanced telecom or information services for telemedicine or the provision of health care for victims of Hurricane Katrina”. 

Applicants not in a Hurricane Katrina relief area that file requests with the Federal Communications Commission under these provisions (see Areas Eligible for Support) should include a copy of that request with their Form 465. 

After filing Form 465, applicants should select a service provider and file Form 466A.
