Documentation / certification requirements

Applicants should file FCC Form 465 (Description of Services Requested and Certification) as soon as possible, but no later than December 13, 2005. 

On Line 29 of the Form 465, all applicants should write, “This is a request for advanced telecom or information services for telemedicine or the provision of health care for victims of Hurricane Katrina”. 

This certification is required to receive this support.
