By December 13, 2005, all health care providers eligible under these temporary rules must file, or if necessary, amend FCC Form 465 (Description of Services Requested and Certification) for Funding Year 2005, to apply for this support.

By March 1, 2006, applicants must file FCC Form 466A for this support.
