Katrina High Cost Certification – Prior to Use of Funds
Any ETC seeking to use some of their high-cost support to assist in disaster
restoration must submit the Katrina High Cost certification prior to the
use of support. USAC requests that all carriers seeking to use some high-cost
support in the more flexible manner contemplated by the Katrina Order, file
an estimate of the support dollars to be used in disaster restoration when
submitting the certification.
Other High Cost Filing Deadlines
Consistent with the Katrina Waiver Order (DA 05-2484, released September 21,
2005) the following high-cost filings must be received no later than January
26, 2006:
- Carriers:
- Mandatory quarterly line count filings for High Cost Model
(“HCM”)
support and Interstate Access Support (“IAS”) for incumbent
and competitive eligible telecommunications carriers (ETCs).
o Mandatory quarterly line counts filings for all High Cost components
received by competitive ETCs.
- Mandatory quarterly line count filings
for High Cost Loop (“HCL”),
Local Switching Support (“LSS”), and Interstate Common Line Support
(“ICLS”) for incumbent ETCs.
- Voluntary quarterly updated line
count and data filings for all other incumbent ETCs.
- Carrier filed use
certifications for HCL support, Safety Net Additive (“SNA”)
support, Safety Valve Support (“SVS”), LSS, and HCM support.
- FCC
Forms LSSc and LSSa – Projected LSS data for 2006 payments.
- FCC Form
509 – ICLS annual true-up data filing.
- FCC Forms LSSc and LSSa – 2004
LSS true-up data.
- State commissions:
- Rate comparability certifications for non-rural carriers.
- State filed
use certifications for HCL support, Safety Net Additive (“SNA”)
support, Safety Valve Support (“SVS”), LSS, and HCM support.
