1.) If I am a USF Contributor in an area that was affected by Hurricane Katrina, can I request an extension to file my 499 form?

A. You may postpone filing the FCC Form 499-Q due November 1, 2005 until January 26, 2006. While notification to USAC is not mandatory, we would appreciate you notifying USAC Customer Service at 1-888-641-8722 if you intent to delay your 499-Q filing to ensure that we know that you are affected. By notifying USAC, this will permit us to suppress the late filing fee for the November 499-Q.


2.) What if I receive late payment fees for my USF Contributions and I am in the affected area and will be delaying one or all of my payments due September 15, 2005 – January 15, 2005 until January 26, 2006?

A. Contact USAC Customer Service at 1-888-641-8722 to discuss any late payment fees assessed on your contributor invoice that is permitted to be made late until January 26, 2005. USAC will reverse any late payment fees covered by the order. By contacting us, we then can also suppress the late payment fees until January 26, 2006.


3.) How will my November 499-Q be calculated if I do not file it until January 26, 2006.

A. If you are in one of the affected areas and do not submit your 499Q by November 1, 2005, USAC will estimate you contribution for the 1st quarter of 2006. Your account will be adjusted in the 2nd quarter of 2006 to reflect your actual filing if received by the January 26, 2006 deadline.


4.) Who can I contact for additional assistance if I have any questions about my FCC Form 499 and USF contributor invoices?

5.) I am requesting temporary Eligible Telecommunication Carrier status to receive Low Income and/or High Cost support, will I need to obtain a Service Provider Identification number (SPIN) in order to receive support payments?

A. To receive payment from USAC, a Service Provider Identification number (SPIN) is required. If you have any existing SPIN but do not participate in Low Income or High Cost, you may simply update your existing SPIN by completing those sections on the FCC Form 498. If you do not have an existing SPIN or wish to establish a new SPIN for this purpose, you may complete the FCC Form 498 to obtain a new SPIN.
