Collections: Appeals Payments
Contributors and service providers that disagree with administrative decisions made by USAC on billing, collection, or disbursement matters may seek an appeal of those decisions from USAC or directly with the Federal Communications Commission (FCC).
In some cases, when an appeal is denied by USAC or the FCC, the contributor or service provider may be required to return funds to USAC.
Payments should be sent to USAC using the Payment Instructions on our website.
For these payments, USAC needs:
- The Service Provider Identification Number(s) (SPINs),
- The Funding Request Number (FRN),
- The Form 471 Application ID Number (471#),
- The Billing Entity Number (BEN),
- The Health Care Provider Number, and
- The Appeal Decision.
